Physical Therapist

Role Summary

Are you a passionate therapy professional looking to escape the high-volume burnout of traditional clinics? We are seeking a dedicated Physical Therapist to join our mission-driven, value-based team. In this deeply rewarding role, you will leave behind the stress of seeing three to four patients an hour. Instead, you will have the time and resources to build long-term relationships with a smaller panel of vulnerable seniors, focusing on comprehensive, restorative care. Working collaboratively alongside an integrated team of clinicians and specialists, you will design and deliver personalized rehabilitative physical therapy plans that maximize patient independence and safety at home. If you want a role that balances clinical autonomy with an unrivaled work-life balance and zero after-hours documentation, we invite you to lead our therapy department!

Duties & Responsibilities

Comprehensive Rehabilitative Care

  • Holistic Assessments: Conduct thorough physical therapy assessments within our state-of-the-art facility and occasionally within patients' homes to identify unique care needs, physical benchmarks, and personal goals.

  • Therapeutic Interventions: Deliver expert treatments including active and passive exercise, muscle re-education, gait training, and functional training for activities of daily living (ADLs).

  • Advanced Modalities: Apply specialized physical therapy treatments as appropriate, including therapeutic massage, heat, sound, water, light, or electrical modalities.

  • Team Care Planning: Actively participate in collaborative team meetings to share clinical insights, establish benchmarks, and build deeply personalized care plans.

  • Supervision & Mentorship: Directly supervise, instruct, and guide Physical Therapy Assistants (PTAs) to successfully implement tailored Functional Maintenance Programs (FMPs).

  • Equipment & Safety Training: Provide clear, empowering training and instruction to staff, patients, and family caregivers on the proper use, safety protocols, and care of therapeutic equipment.

  • Compliant Documentation: Deliver and document all clinical interventions in real time, following established department guidelines and ensuring all administrative work is wrapped up before your shift ends.

  • Other duties as assigned.

Required Qualifications

  • Licensure: Valid California Physical Therapy License

  • Education: Bachelor’s degree in Physical Therapy from an accredited program.

  • Life Support: Ability to obtain CPR and First Aid certifications within 180 days of hire (certifications in hand upon hire preferred).

  • Core Competencies: Exceptional clinical reasoning, strong interpersonal communication skills, and a collaborative spirit suited for a multidisciplinary clinical team.

Desired Qualifications

  • Advanced Degrees: Master’s or Doctoral degree (DPT) in Physical Therapy preferred.

  • Geriatric Focus: Two (2) or more years of experience in a clinical setting working directly with a frail, elderly, or specialized senior population is preferred.

  • Clinical Background: Previous hands-on experience across outpatient, acute care, inpatient rehab, or skilled nursing facilities.

Location and Work Type

  • Work Type: Full-Time, Permanent Position.

  • Work Setting: On-site clinical facility care, integrated with occasional community-based home visits (minimal local travel required).

  • Schedule: Consistent, stable daytime schedule providing an exceptional alternative to traditional, high-burnout therapy roles.

  • Location: La Quinta, California (Dedicated office space available on-site).

Compensation, Benefits & Perks

  • True Work/Life Balance: Enjoy 17 days of Personal Time Off (PTO), 12 paid holidays observed annually, and 6 designated sick days.

  • Comprehensive Healthcare: Immediate access to premium medical, dental, and vision insurance coverage options.

  • Financial Security: 401(k) retirement savings program equipped with a competitive company match.

  • Total Rewards: A robust total compensation package including stable base pay, performance bonus opportunities, and continuous professional development paths.
