Key Responsibilities

  • Evaluate patients and develop individualized treatment plans.

  • Provide skilled physical therapy services in accordance with physician orders and care plans.

  • Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.

  • Collaborate with interdisciplinary team members to coordinate and optimize patient care.

  • Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.

  • Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.

  • Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.

  • Assist with discharge planning and recommendations for continued care.

  • Recommend and coordinate appropriate durable medical equipment (DME).

  • Ensure a safe treatment environment by monitoring and reporting equipment maintenance needs.

Qualifications

  • Current Physical Therapist (PT) license in good standing.

  • Strong communication, organization, and interpersonal skills.

  • Passion for delivering exceptional patient-centered care.

  • Ability to work effectively within a multidisciplinary team.
