Job Summary:

Evaluates and treats all referred Physical Therapy patients according to physician orders. Home Care: Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.

EDUCATION DESCRIPTION:

Degree in Physical Therapy required. Wisconsin licensed Physical Therapist or eligible required. If eligible, must obtain Wisconsin license within 3 months of hire.

EDUCATION DESCRIPTION:

At least one year experience in a medical setting preferred.

SPECIAL SKILLS DESCRIPTION:

Good communication skills required.

LICENSURE DESCRIPTION:

CPR certification is required.
