Key Responsibilities of the Physical Therapist- Conduct home health therapy services with adult patients
- Manage chronic diseases, wound care, and advanced clinical procedures including IV, pleurx, and wound vac
- Utilize Homecare Homebase EMR system for accurate charting
- Provide disease education and create maintenance notes for geriatric/chronic disease management
Requirements of the Physical Therapist- Bachelor's Degree in Physical Therapy
- Minimum of 1 year of experience in home health settings
- Active State Licensure and BLS certification
- Driver's License and proof of insurance
- Proficiency in Homecare Homebase EMR and OASIS-E assessment tool
