Position Overview

We are seeking a licensed Physical Therapist to join a reputable Skilled Nursing and Rehabilitation Facility in Roseville, California. This facility provides post-acute rehabilitation, long-term care, and memory care services to adult and senior patients. The Physical Therapist will play a key role in evaluating patients, developing individualized treatment plans, and delivering evidence-based interventions to support functional recovery and mobility.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments

  • Develop and implement individualized plans of care based on patient needs and goals

  • Provide therapeutic interventions focused on mobility, strength, balance, and functional independence

  • Monitor patient progress and modify treatment plans as clinically indicated

  • Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and medical providers

  • Educate patients and families on treatment goals, progress, and home exercise programs

  • Maintain accurate, timely documentation in compliance with facility policies and regulatory requirements

  • Utilize evidence-based practice standards to support optimal patient outcomes

Qualifications

  • Active California Physical Therapist license in good standing

  • Prior experience in a Skilled Nursing Facility or post-acute setting preferred

  • Strong clinical assessment and treatment planning skills

  • Excellent communication and interpersonal abilities

  • Ability to manage documentation efficiently and accurately

  • Commitment to delivering high-quality, patient-centered care
