Whitney Oaks Care Center is seeking a licensed Physical Therapist (PT) to join our dedicated rehab team in Carmichael, CA. We are a skilled nursing and post-acute care facility focused on delivering high-quality rehabilitation services to help patients regain mobility, strength, and independence.

Physical Therapist (PT) Responsibilities
• Evaluate residents and patients and develop individualized physical therapy treatment plans • Provide skilled therapy interventions to improve mobility, strength, balance, endurance, and functional independence • Collaborate with occupational therapists, speech therapists, nurses, and physicians to support optimal patient outcomes • Educate patients and caregivers on safety, mobility strategies, fall prevention, and home exercise programs • Complete accurate and timely documentation in compliance with regulatory and facility standards
Qualifications
• Current California Physical Therapist license required • Experience in a skilled nursing facility (SNF), rehab hospital, post-acute care, or outpatient setting preferred • Strong clinical, communication, and interdisciplinary teamwork skills • Passion for rehabilitation therapy, patient recovery, and working with older adults
