Schedule & Assignment Details:

• Location: CA 95816

• Start: ASAP

• Duration: 13-week contract

• Hours: 36 hours/week

• Pay: $90/hr

• Setting: Skilled Nursing Facility (SNF)

• Type: Travel or Permanent option available

Position Summary & Responsibilities:

• Evaluate, plan, and implement individualized PT treatment plans for SNF residents

• Provide therapeutic interventions to improve mobility, strength, balance, and function

• Document patient progress and update care plans in compliance with facility standards

• Collaborate with interdisciplinary team (RN, OT, SLP, MD) to support patient outcomes

• Educate patients and caregivers on mobility, safety, and rehabilitation goals
