Fountain Care at Sunset Hills is seeking a compassionate and motivated Physical Therapist (PT) to join our dedicated in-house rehabilitation team. We are currently hiring for both Full-Time and Per Diem opportunities. This is an excellent opportunity to provide high-quality, patient-centered care in a supportive skilled nursing environment while helping residents achieve their highest level of independence and mobility.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide evidence-based therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
• Establish realistic treatment goals and monitor resident progress.
• Collaborate with physicians, nursing staff, rehabilitation team members, residents, and families to coordinate patient care.
• Educate residents and caregivers on exercise programs, mobility techniques, fall prevention, and the safe use of assistive devices.
• Maintain accurate, timely documentation in accordance with state, federal, and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Ensure compliance with all regulatory and professional standards.

Qualifications

• Current Missouri Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Skilled nursing facility or rehabilitation experience preferred, but new graduates are encouraged to apply.
• Excellent communication, interpersonal, and clinical decision-making skills.
• Ability to work effectively within a multidisciplinary team.
• Commitment to providing exceptional, resident-centered care.
