Physical Therapist

Job Description

We are seeking a Physical Therapist for a skilled nursing and rehabilitation center in Salinas, CA. This facility provides post-hospital rehab, long-term care, restorative programs, and support for medically complex residents.
You will assess patient needs, create personalized treatment plans, and deliver therapeutic interventions aimed at improving strength, mobility, safety, and overall function. The setting offers interdisciplinary collaboration and flexible scheduling.

Responsibilities

• Perform evaluations and functional mobility assessments.

• Develop customized treatment plans based on patient needs.

• Provide therapeutic interventions including gait, transfer, and strengthening programs.

• Reassess patients and modify plans accordingly.

• Educate families and caregivers.

• Document all care per state and facility guidelines.

• Collaborate closely with RN, PT, OT, ST, and case managers.

• Participate in interdisciplinary planning and discharge support.

Experience / Qualifications

• Active California PT license.

• SNF or post-acute experience preferred.

• Strong assessment and treatment-planning abilities.
