Responsibilities
- Perform assessments and develop individualized care plans for home health patients.
- Provide therapeutic interventions and educate patients and families on exercises and safety.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document patient progress and maintain accurate clinical records.
Qualifications
- Active Physical Therapist license in the state of Illinois.
- CPR certification (AHA or American Red Cross).
- Minimum 1 year of experience in a healthcare setting.
- Reliable transportation for home visits.
- Strong communication skills and commitment to quality patient care.
