Physical Therapist (PT) – [Outpatient Care ] [South Tampa | Monday–Friday] Work in a supportive assisted living community where you can build meaningful relationships with your patients and see their progress over time. Today, TGH Home Care powered by VNA of Florida brings that trusted experience and compassionate care to Tampa General Hospital patients and the Tampa Bay community.Join a team backed by the strength of Tampa General Hospital and VNA's 50-year legacy of care, where your expertise matters and your work makes a meaningful difference in the lives of patients and families. What You'll DoProvide skilled physical therapy services under Medicare Part B to residents in an assisted living community.Complete comprehensive physical therapy evaluations and assessments of mobility, strength, balance, coordination, gait, and functional abilities.Develop individualized treatment plans based on each patient's needs, goals, and level of function.Provide therapeutic interventions designed to improve strength, mobility, balance, endurance, coordination, and overall functional independence.Evaluate patients' progress and modify treatment plans as appropriate to achieve optimal outcomes.Help patients safely perform activities of daily living and maintain their independence within the community.Identify fall risks and functional limitations and implement appropriate interventions to improve safety and mobility.Educate patients, families, caregivers, and facility staff on exercises, mobility strategies, fall prevention, and recommendations for continued progress.Coordinate care with physicians, nurses, occupational and speech therapists, facility staff, caregivers, and other healthcare professionals.Communicate changes in patient condition and progress toward goals with the appropriate members of the care team.Participate in interdisciplinary care planning and collaborate with the assisted living community to promote continuity and quality of care.Maintain accurate and timely clinical documentation, including evaluations, treatment notes, progress reports, plans of care, and care coordination.Demonstrate strong customer service and build positive relationships with patients, families, facility staff, and healthcare partners.Participate in ongoing education and professional development to maintain clinical knowledge and provide high-quality care.Be self-motivated, growth-minded, and committed to delivering an exceptional patient experience. What You'll NeedActive Florida Physical Therapist (PT) licenseGraduate of an accredited Physical Therapy programStrong clinical assessment and treatment skillsKnowledge of evidence-based physical therapy practicesStrong communication, organization, and critical-thinking skillsAbility to work independently while collaborating effectively with a multidisciplinary teamCommitment to compassionate, patient-centered careExcellent customer service and interpersonal skillsSelf-motivated and committed to continuous professional growthExperience with older adults, geriatrics, outpatient therapy, assisted living, skilled nursing, or home health is a plus Why You'll Love Working Here Monday–Friday, 8:00 a.m.–5:00 p.m. scheduleConsistent facility-based patient populationCompetitive pay and benefits for eligible positionsSupportive clinical and leadership teamsFree CEUs and professional development opportunitiesCareer growth and advancement opportunitiesBe part of a trusted nonprofit organization with 50 years of compassionate care Apply TodayIf you're passionate about helping others and want to build a rewarding career with a respected home health organization, we'd love to hear from you.VNA of Florida 50 Years of Compassionate Care The Heart of Home Care Equal Opportunity Employer | DFWhttps://info.flclearinghouse.com/TGH1
