Post Rehabilitation Services is seeking a full-time Physical Therapist to help skilled nursing residents rebuild physical abilities, overcome mobility barriers, and prepare for their next stage of care.What You'll DoAssess residents' movement, strength, balance, and functional abilities to identify treatment priorities.Design individualized plans of care that account for medical precautions, previous function, and resident goals.Provide therapeutic exercise, gait training, and task-specific practice for transfers and daily mobility.Evaluate assistive device needs and train residents and caregivers in safe movement strategies.Measure progress, update treatment plans, and provide clinical direction to PTAs.Complete evaluations and treatment records while partnering with nursing and rehabilitation colleagues on discharge recommendations.What We Offer$45–$50 per hour.Medical, dental, and vision insurance.401(k).CEU reimbursement.Parental leave and disability insurance.Unlimited referral bonus, with up to $1,000 per successful referral.Support from therapy leadership and collaboration with fellow clinicians.QualificationsActive Connecticut Physical Therapist license.Strong clinical judgment, assessment, and treatment planning skills.Effective communication and timely, accurate documentation.Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.Apply today to join Post Rehabilitation Services at Civita Care Center West River in Milford.
