Physical Therapist (PT) – Full-TimeWilliamsville Suburban Care Center | Williamsville, NY$38–$44/hour | Full-Time | Benefits AvailablePost Rehabilitation Services is seeking a Physical Therapist to join the rehabilitation department at Williamsville Suburban Care Center. This full-time position combines individualized treatment with ongoing care coordination, helping skilled nursing residents improve physical function and work toward their recovery goals.What You'll DoAssess residents' movement, strength, balance, pain, and functional limitations to determine physical therapy needs.Develop personalized plans of care with goals for daily mobility and safe participation in activities.Provide therapeutic exercise, gait training, transfer practice, and other clinically appropriate interventions.Educate residents and caregivers on mobility devices, fall prevention, and techniques for continued progress.Reassess function, adjust treatment plans, and supervise PTAs participating in resident care.Maintain complete clinical documentation and collaborate with nursing and other therapists on care transitions and discharge planning.What We Offer$38–$44 per hour.Medical, dental, and vision insurance.401(k).CEU reimbursement.Parental leave and disability insurance.Unlimited referral bonus, with up to $1,000 per successful referral.Support from therapy leadership and collaboration with an interdisciplinary care team.QualificationsActive New York Physical Therapist license.Strong clinical reasoning, evaluation, and treatment planning skills.Effective communication and timely, accurate documentation.Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.Apply today to build your physical therapy career at Williamsville Suburban Care Center with Post Rehabilitation Services.
