Physical Therapist - Home Health Location: Sterling Heights, MI Pay Rate: $75,000 to $95,000 per year
Position Overview A growing healthcare organization is seeking a dedicated and compassionate Physical Therapist to join a dynamic home health team in Sterling Heights, Michigan. Key Responsibilities
Conduct thorough initial evaluations and develop individualized treatment plans for patients recovering from surgery, injury, stroke, or managing chronic conditions
Deliver skilled physical therapy services in patients' private residences throughout the Sterling Heights and surrounding communities
Establish measurable, functional goals in collaboration with patients and their families
Monitor and document patient progress accurately and in compliance with all regulatory requirements
Communicate regularly with referring physicians and the broader care team to ensure coordinated, high-quality care
Educate patients and caregivers on home exercise programs, safety strategies, and fall prevention techniques
Manage a caseload efficiently while maintaining excellent standards of clinical care and documentation
Participate in case conferences and contribute to ongoing quality improvement initiatives
Requirements
Active Michigan Physical Therapist license in good standing
Graduate degree from an accredited physical therapy program
Minimum of one year of clinical experience, preferably in home health or a related setting
Valid driver's license and reliable personal transportation
Strong working knowledge of Medicare home health regulations and documentation standards
Demonstrated ability to work independently with excellent time management and organizational skills
Proficiency with electronic medical records systems
Outstanding interpersonal and communication skills with a patient-first mindset
Preferred Qualifications
Experience in a related field is preferred
