Physical Therapist

Role and Responsibilities 

A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physician’s orders.  As a per diem employee, wages will be earned for the completion of a task as stated in the HCR pay practice policy.

Essential Functions 

• Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures. 
• Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment. 
• Recommend modification of plan of care to physician, as needed. 
• Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date. 
• Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence. 
• Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.). 
• Include and educate the patient and caregiver(s) regarding treatment plan and goals.
