Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Colorado state PT license required to start the assignment.
  • BLS certification required from AHA or ARC.
  • Experience in home health settings preferred.
  • Familiarity with HCHB electronic medical record preferred.
  • Ability to manage a territory in El Paso County and drive regularly. Mileage expectation up to approximately 200 miles per week.
  • Experience working in a care coordination model alongside a PTA preferred.


Preferred Qualifications

  • Prior travel or per-diem home health experience.
  • Experience collaborating with PTAs in a caseload or care coordination model.


Assignment Details

  • M-F.
  • Territory coverage: El Paso County.
  • EMR: HCHB.
  • Expected mileage: approximately 200 miles per week.
  • Work with a PTA in a care coordination model.

Impacting Patient Care
