    Job Description

    Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist position in Jackson, TN providing home health services where you will help patients recover from injuries and manage chronic conditions while working in patients' homes. Provide comprehensive assessments, develop and implement individualized plans of care, instruct patients and caregivers on safe use of assistive devices, and collaborate with interdisciplinary teams to achieve functional goals.     Minimum Requirements

    • Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an American Physical Therapy Association–approved program.
    • Current state license as a Physical Therapist required before the start of the assignment.
    • Proof of current CPR certification required.
    • Current driver’s license, reliable transportation, and auto insurance required.
    • 1–2 years of experience as a Physical Therapist in acute care or rehabilitation settings required.
    • Experience with durable medical equipment and ability to order and adapt home rehabilitative equipment required.
    • Ability to perform physical tasks including lifting, bending, pushing, pulling, prolonged standing and walking required.
    • Ability to travel locally and work a flexible schedule required.
    • Must read, write, and speak fluent English.
    • Preferred: Previous home health experience.


    Preferred Qualifications

    • Bachelor of Science in Physical Therapy from an APTA-approved program noted as preferred when applicable.
    • Previous home care experience preferred.


    Assignment Details

    • Works in patient homes providing home health physical therapy services.
    • Must be able to travel locally to visit patients.

    Impacting Patient Care

    Provide direct physical therapy services that restore or maintain patient well-being in the home environment. Perform clinical assessments, develop and revise plans of care, document progress notes and phone orders, and collaborate with physicians and interdisciplinary teams. Educate patients and families on therapeutic procedures and safe use of assistive devices. Apply infection control and universal precautions. Supervise other team members when applicable. Maintain required continuing education and demonstrate professional competency.
