    Description

    Position: Physical Therapist 

    Salary Range: $40.00 - $45.00 per hour

    Hours: 40 hours per week, Monday - Friday with a weekend rotation

    Facility: 115 bed skilled nursing facility offering long-term and short-term rehab

    Job Description:

    We are seeking a highly skilled and compassionate Physical Therapist to join our team at our 115 bed skilled nursing facility. Our facility offers both long-term care and short-term rehabilitation services to meet the needs of our diverse patient population.

    As a Physical Therapist, you will be responsible for evaluating and treating patients with a variety of conditions, including but not limited to orthopedic, neurological, and cardiopulmonary disorders. You will develop individualized treatment plans based on each patient's specific needs and goals, and work closely with other members of the healthcare team to ensure the best possible outcomes for our patients.

    Key Responsibilities:

    - Conduct initial evaluations and ongoing assessments to determine patient needs and develop treatment plans

    - Provide hands-on therapy and exercises to improve mobility, strength, and function

    - Educate patients and their families on proper techniques and exercises to continue at home

    - Monitor and document patient progress and make necessary adjustments to treatment plans

    - Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care for patients

    - Maintain accurate and up-to-date patient records

    - Participate in interdisciplinary team meetings and provide input on patient care plans

    - Stay current with industry developments and advancements in physical therapy techniques and equipment

    Qualifications:

    - Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program

    - Current state license to practice as a Physical Therapist

    - Minimum of 1 year of experience in a skilled nursing or rehabilitation setting preferred

    - Strong communication and interpersonal skills

    - Ability to work independently and as part of a team

    - Compassionate and patient-focused approach to care

    - Basic computer skills for documentation and communication purposes
