A Home Health Physical Therapist is responsible for evaluating and treating patients in their homes, developing individualized treatment plans to improve mobility, manage pain, and restore function following injury, surgery, or illness, by utilizing exercises, manual therapy, and other therapeutic modalities, while educating patients and their families on self-care practices and coordinating with other healthcare team members to ensure optimal patient care.
