Physical Therapists

Per Diem, Days/Occasional Weekends and Holidays.

Job Description:

-Consults with patients/reviews medical history and referrals on individuals’ physical condition and symptoms; diagnoses movement dysfunction; develops and implements a treatment plan.

-Serves as a member of a treatment team; attend care plan meetings, communicates physical therapy treatment plans to family members.

-Administers physical therapy treatment programs and services to individuals with more severe or acute physical disabilities.

-Assist individuals with the use of equipment such as wheelchairs or walkers.

-Teach and coach therapeutic exercise and stretches to improve movement.

-Observe and review individuals’ progress and adapt treatment accordingly.

-Supervise and train Physical Therapy Assistant, review and cosign paperwork.

-Evaluates and initiates skilled Physical Therapy treatments in accordance with provider orders.

-Maintains communication with providers.

-Performs and completes paperwork and billing in a timely manner.

-Participates with interdisciplinary communication and collaboration.

-Updates Residents’ care plan when changes are noted.

-Participates with discharge planning and home assessments.

-Participates with identifying and ordering equipment needed.

-Professional interaction with residents, coworkers, visitors, and families.

-Attends to responsibilities of the Director of Rehab in their absence.
