Hayward Gardens Post Acute is seeking a compassionate, motivated, and dedicated Physical Therapist Assistant (PTA) to join our in-house rehabilitation team!

Under the direction and supervision of a licensed Physical Therapist, you will provide hands-on therapy services to help residents improve mobility, strength, balance, coordination, and overall functional independence.

What You'll Do

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Assist residents with therapeutic exercises and activities to improve strength, mobility, balance, coordination, and endurance.
• Help residents with transfers, ambulation, positioning, and functional mobility.
• Monitor resident responses to treatment and communicate progress or concerns to the supervising Physical Therapist.
• Document therapy treatments and resident progress accurately and in a timely manner.
• Educate residents and caregivers on exercise programs, mobility techniques, safety strategies, and fall prevention.
• Assist with recommendations and training related to adaptive and mobility equipment.
• Collaborate with nursing staff, physicians, residents, families, and interdisciplinary team members.
• Participate in care conferences and rehabilitation meetings as needed.
• Maintain a safe, clean, and organized therapy environment.
• Follow infection control procedures and facility safety policies.

Qualifications

• Current and active California Physical Therapist Assistant (PTA) license in good standing.
• Graduate of an accredited Physical Therapist Assistant program.
• New graduates are encouraged to apply!
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.
• Strong communication and interpersonal skills.
• Compassionate, professional, and resident-centered approach to care.
• Ability to work effectively as part of an interdisciplinary team.
• Strong organizational skills and attention to detail.
