    Job Description

    WellTech Partners is assisting a home health agency in their search for a dedicated and skilled full-time Physical Therapist to join their team in the Detroit metro area, specifically serving Detroit and Oakland County. This opportunity is with a well-established home care center that has been in service for 15 years, committed to providing high-quality patient care with a focus on flexibility and professionalism.

    Responsibilities:

    • Provide physical therapy services in a home care setting.
    • Manage a caseload of approximately 5-7 patients per day, adjusting for location, drive time, and patient complexity.
    • Conduct sessions that vary in length (30-60 minutes) based on individual patient needs and tolerance.
    • Independently assess and manage patient conditions, including identifying and responding to critical situations such as CHF or COPD crises, given the unsupervised home setting.
    • Develop and implement treatment plans for patients requiring basic rehab, disease management, joint care, orthopedics (including post-joint replacement for shoulders, hips, and knees), and post-surgical care.
    • Maintain timely and accurate documentation.
    • Communicate effectively with patients, families, and the healthcare team, and actively participate in case conferencing.
    • Manage your own schedule efficiently, clustering patients geographically to minimize travel time.

    Qualifications:

    • Possession of a Doctor of Physical Therapy (DPT) degree is preferred; however, candidates with a PT degree and significant clinical experience will be considered.
    • Current and active Physical Therapist license in the state of Michigan.
    • Minimum of [Insert specific number, e.g., 1-2] years of clinical experience, with preference for experience in a home care setting. New graduates are not preferred due to the independent nature of the role.
    • Proficiency in basic rehab techniques, disease management, orthopedic conditions, and post-surgical care.

    • Ability to work independently and make autonomous clinical judgments.

    Compensation:
