In 1995, in Baytown, Texas, a dedicated, community-minded Registered Nurse –determined to make a quality difference in home health care—started her own health care agency. Signature boasts outstanding team members with longevity, who have advanced in their careers through education and growth at work. To give one example: in 2007, one team member started her career with Signature Health Services as a field LVN, giving excellent hands-on care to our patients. As her passion began to grow for home health, she furthered her education while still working, obtaining her RN in 2011. She transitioned into being one of our top producing RN field nurses specializing in wound care and trained under one of our Medical Directors who is a Certified Wound Specialist through the American Board of Wound Healing. She has advanced to become our Director of Nursing. That’s how we motivate our best to embrace growth, excellence, and teamwork.

All aspects of patient care are handled internally and onsite. That encompasses our outstanding scheduling department, therapy coordinator, intake, billing, orders, clerical coordinator, and director of nursing. Signature Health Services hosts a team of outstanding, accomplished, and tenured Supervising Registered Nurses who are in-house and ready to assist patients, caregivers, physicians or their staff, including through our after-hours, 24/7 on-call service. Signature operates under a case management model to provide continuity of care for our patients. What this means is that one of our in-house supervising nurses is responsible for a group of patients, in addition to that same group of patients being assigned to a primary field RN and LVN to ensure closely-monitored and excellent healthcare.
