Location: Central Riverside

$5,000 Sign-On Bonus: Paid out quarterly over the course of 1 year.

Job Summary: The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.

Qualifications:

• Graduate of an AMA-approved (or equivalent) PT program
• Licensed PT in the state of practice
• 2+ years experience preferred
• 1 year of home health experience required
• Current CPR certification
• Ability to work independently
• Strong verbal and written communication skills
• Valid driver's license and insured vehicle
• Meets pre-employment health requirements

Responsibilities:

• Provide PT services as ordered by physician
• Assist with patient evaluations and Plan of Care
• Document and report patient progress
• Educate patients, families, and staff
• Evaluate home environment as needed
• Maintain accurate clinical documentation

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
