1. CV (minimum of 3 years experience in the last 5 years)
2.licensed healthcare professional who works with physicians to provide patient care in a hospital setting.
3.The license can be from any of the 50 states and or territories
4. BLS Certification or ACLS (check expiration date)
5. Must have a valid, unrestricted medical license in at least 1 state, territory, or commonwealth of the U.S.
6. Immunizations / Covid 19
7. Copy of CMEs
8. National Provider Identifier [NPI]
9. DEA Certificate
10. Must be a US Citizen or Legal Resident
11. Nights 4:00PM 8:00 AM Monday through Friday [including Federal Holidays (24 hours), 24-hour or 48-hour weekend
12. coverage, and any other mutually agreed upon hours. This will include day shifts on holidays, day or evening shifts on weekends, and coverage in UC on occasion.
