Role Overview: Part‑Time Physician — Charlottesville, VA

We are seeking a dedicated and experienced Physician to provide part‑time primary and acute medical services to residents within post‑acute and long‑term care settings in Charlottesville. The successful clinician will exercise strong clinical judgement, collaborate closely with facility teams, and manage both routine and complex medical issues while promoting resident‑centered goals and quality outcomes.

Primary Responsibilities

  • Provide timely on‑site assessment, diagnosis and management of acute clinical changes for residents in post‑acute and long‑term care settings

  • Manage chronic medical conditions with a focus on preserving function, reducing hospitalizations and optimizing quality of life

  • Perform comprehensive history and physical examinations, develop individualized care plans and set measurable goals in collaboration with residents, families and facility staff

  • Order, interpret and act upon diagnostic tests; initiate treatments and arrange specialist referrals as necessary

  • Provide safe and effective medication management including reconciliation, monitoring and deprescribing when appropriate

  • Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)

  • Collaborate with nursing and interdisciplinary teams through care‑planning meetings, rounds and family conferences to ensure coordinated, person‑centered care

  • Support facility quality and safety initiatives such as infection prevention, falls reduction and medication safety

  • Provide clinical leadership and education to facility staff as needed

Required Qualifications and Professional Skills

  • MD or DO degree from an accredited medical school

  • Active and unrestricted Virginia medical license and current DEA registration as required

  • Board certification in Internal Medicine, Family Medicine or Geriatric Medicine preferred

  • Experience working in post‑acute, long‑term care, geriatric or similar clinical settings preferred

  • Proficiency with Electronic Medical Record systems and accurate clinical documentation

  • Strong clinical judgement, excellent communication skills and the ability to work collaboratively within multidisciplinary teams

  • Commitment to compassionate, person‑centered care, cultural sensitivity and ethical practice

  • Current Basic Life Support (BLS) certification required; additional certifications (ACLS, PALS) desirable depending on facility needs
