Legacy Care is seeking a compassionate, clinically strong physician to provide PRN coverage for our Richmond-area post-acute care practice. This is an opportunity to deliver high-quality care with flexibility and autonomy while supporting medically complex adult and geriatric patients across post-acute and long-term care settings.

You will work alongside advanced practice providers, facility leadership and interdisciplinary care teams to provide responsive, person-centered care in skilled nursing, rehabilitation, long-term care and assisted living environments.

What You’ll Do

• Provide PRN coverage for assigned Richmond-area facilities

• Deliver onsite primary, acute and chronic care to post-acute and long-term care residents

• Evaluate changes in condition, acute concerns and complex clinical cases

• Collaborate with nurse practitioners, nursing teams, facility leadership and other care professionals

• Partner with patients and families to establish person-centered goals of care

• Support appropriate utilization, quality improvement and regulatory compliance initiatives

• Complete timely, accurate clinical documentation

• Promote smooth transitions of care across the healthcare continuum

What We’re Looking For

• MD or DO with an active, unrestricted Virginia medical license

• Active DEA registration

• Board certification or board eligibility in Internal Medicine, Family Medicine or Geriatric Medicine

• Experience in skilled nursing, long-term care, geriatrics, hospital medicine, primary care or post-acute medicine preferred

• Strong clinical decision-making, communication and collaboration skills

• Ability to work independently while partnering effectively with facility-based care teams

• Ability to travel locally among assigned Richmond-area facilities

Schedule and Compensation

• Schedule: PRN, based on practice and facility needs

• Coverage: Flexible weekday and/or weekend opportunities may be available

• Clinical compensation: Daily rate

• Employment structure: W2

• Anticipated start: As soon as credentialing is completed
