As a Federally Qualified Health Center (FQHC) and Social Services organization, we proudly serve diverse communities in Santa Clara County delivering high-quality, culturally responsive care to those who need it most. AACI is seeking a dedicated and compassionate Primary Care Physician (MD or DO) to Provide outpatient medical services from 320-40 hours per week.

The listed range is a base compensation for full time (1.0 FTE) clinician based on years of licensure.

What You'll Do

  • Provide comprehensive, patient-centered outpatient primary care to patients of all ages.
  • Conduct physical exams, interpret diagnostics, and develop individualized treatment plans. No OB.
  • Collaborate with a multidisciplinary team, including behavioral health and community wellness staff.
  • Support chronic disease management through patient education and self-care guidance.
  • Coordinate specialty referrals and inpatient care when needed.
  • Completing patient forms including DME, DMV, IHSS, EDD/disability, school, related medical forms and In-basket management
  • Participate in quality improvement initiatives, peer reviews, and clinical audits.
  • Collaborate with a multilevel, multidisciplinary team of clinical and non-clinical staff to provide outpatient services as scheduled, following appropriate quality and productivity standards of care.
  • Collaborate with Behavioral Health and Wellness colleagues to integrate health, behavioral health, and wellness care for all AACI patients.
  • Participate in clinical quality improvement processes, peer reviews and certifications.
  • On a rotational basis, serve as an on-call physician for afterhours support.
  • May participate in outside events such as health fairs and other community health related activities.
  • Medical Doctor or Doctor of Osteopathic Medicine Degree from an accredited medical institution
  • Board Certified in Family Medicine, Internal Medicine, or related specialty
  • Current, valid, unrestricted professional license to practice in the state of California
  • Active and unrestricted DEA License
  • Minimum 2 years of primary care clinical experience
  • Current BLS certification
  • Ability to manage a full-time patient panel of approximately 1,500 patients
  • Passion for serving underserved and multicultural communities
  • Strong collaborative mindset and commitment to team-based care
  • High level of written skills
  • Ability to communicate well with people of diverse cultural professional and experiential backgrounds
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team
  • English language proficiency required. Proficiency in another language a plus
