We are currently looking for a Family Medicine nurse practitioner ANDOR physician assistant to join our Family Medicine private practice in Lincoln CA a suburb of Sacramento. Key Responsibilities Comprehensive Primary Care Provide preventive care including annual physicals well-child visits immunizations and sick visits for both children ages 6 and older and adults. Acute Care Services Offer urgent care pediatric care short-term stabilization and post-hospital follow-up care designed to keep patients safe independent and out of the hospital. Chronic Disease Management Manage acute and chronic conditions including providing medication therapy management diet and nutrition counseling and mobilityhome medical equipment assessments. Geriatric & Palliative Care Provide specialized adult primary care and patient observation tailored to our senior citizen population. Collaborative Practice Work collaboratively with our clinical team and network of specialists to ensure comprehensive continuity of care. Work Life Balance Predictable work schedule flexible full or part-time option no after hours practice call.
