Practice Quality Liaison

Job Summary

The Practice Quality Liaison serves as the primary field-based partner to assigned practices, supporting improvement in quality performance, care gap closure, and value-based care outcomes. This role bridges population health strategy with day-to-day practice operations, enabling clinicians and practice teams to succeed in quality measurement, workflow optimization, and patient engagement.

The liaison uses data-driven insights, strong practice relationships, and hands-on coaching to drive measurable improvements in HEDIS/quality metrics, risk adjustment performance, and overall patient outcomes.

This role combines strategic relationship building and coordination with direct execution and partnership to translate goals into measurable practice outcomes.

Responsibilities

Practice Partnership & Engagement

• Serve as the main point of contact for assigned practices regarding quality and population health initiatives
• Build trusted relationships with providers, practice leaders, and care teams
• Facilitate alignment between organizational quality goals and practice-level execution
• Participate in practice meetings, huddles, and workflows to provide real-time support

Quality Performance Improvement

• Analyze and interpret quality data (e.g., HEDIS, CMS Star measures, internal KPIs)
• Identify care gaps and prioritize opportunities for improvement
• Develop and implement action plans to improve quality scores and outcomes
• Monitor progress and report performance trends to leadership and practices

Workflow Optimization & Enablement

• Assess practice workflows and identify barriers to quality performance
• Recommend and support implementation of standardized workflows and best practices
• Collaborate with clinical and operational teams to embed quality into daily routines
• Support use of EHR tools, registries, and dashboards for care gap management

Care Gap Closure & Population Health Support

• Drive closure of preventative and chronic care gaps (e.g., screenings, vaccinations, chronic disease management)
• Support outreach strategies, including patient lists, recalls, and engagement campaigns
• Partner with care management teams to coordinate quality, gap closure, and high-risk patient interventions that reduce overall cost of care

Education & Coaching

• Provide training and coaching to providers and staff on quality measures, documentation, and coding
• Deliver targeted education based on practice performance and identified gaps
• Reinforce evidence-based guidelines and population health strategies

Data, Reporting & Insights

• Leverage dashboards and analytics to track practice performance
• Translate data into actionable insights for practice teams
• Provide regular performance updates and recommendations to leadership
• Prepare executive level presentations for quality updates

Collaboration & Cross-Functional Work

• Partner with clinical quality, data analytics, care management, and operations, and other network teams
• Align practice-level efforts with organizational population health priorities
• Support value-based care initiatives and contract performance goals
