Job Summary
The Practice Quality Liaison serves as the primary field-based partner to assigned practices, supporting improvement in quality performance, care gap closure, and value-based care outcomes. This role bridges population health strategy with day-to-day practice operations, enabling clinicians and practice teams to succeed in quality measurement, workflow optimization, and patient engagement.
The liaison uses data-driven insights, strong practice relationships, and hands-on coaching to drive measurable improvements in HEDIS/quality metrics, risk adjustment performance, and overall patient outcomes.
This role combines strategic relationship building and coordination with direct execution and partnership to translate goals into measurable practice outcomes.
