Position: Nurse Practitioner - Family Practice (1 opening)

Deliver comprehensive primary care in the Bronx, managing acute/chronic conditions for diverse families in a community health setting.

Key Responsibilities:

• Conduct physicals, vaccinations, sick visits, and wellness screenings
• Manage diabetes, hypertension, asthma, and preventive care
• Order labs/imaging, prescribe medications, and coordinate referrals
• Provide patient education and chronic disease counseling

Qualifications:

• FNP certification with active NY NP license
• Primary care experience preferred; bilingual Spanish advantageous
• BLS; EHR proficiency required
