Position: Primary Care Nurse Practitioner, Bilingual

Candidate must reside in LA or be willing to relocate.

Schedule: 40 hrs p/wk, no on-call. Schedule varies.

Salary: $130k - $183k p/yr

Position Information:

This is an outstanding opportunity for a mission-driven team player who is passionate about enabling seniors to meet their goals and live the independent lives they deserve. In this role, you will serve on an interdisciplinary team (IDT) and provide direct medical care to PACE participants. You will work collaboratively with the IDT to create and drive forward a care plan that improves health outcomes for your panel of participants. In this role, you will meet participants where they are whether that be in the center, clinic, home, virtually or an external site of care.

Qualifications:

• Master's degree or higher
• Nurse Practitioner (4+ years)
• 4+ years of experience in ANY of the following:
• Geriatric Care
• Geriatric/LTC
• Valid American Heart Association Basic Life Support (AHA BLS)
• Valid Nurse Practitioner (NP) in the state of California (CA)
