What you will accomplish in this role:
- Performing history and physical examinations
- Documenting visits in electronic medical record in a timely manner
- Educating patients on their health conditions in a way they can understand.
- Following up with patients more frequently for effective condition management.
- Utilizing your support team to complete standing orders
- Ordering diagnostic tests based on assessments, developing treatment plans, prescribing drugs and other therapies as needed
- Making referrals as necessary, consulting with supervising medical provider when appropriate.
- Maintaining patient confidentiality
- Participating in meetings
- Working collaboratively with other members of the medical team
- Collaborating with the management team on quality improvement initiatives
- Having an open mind to feedback, different perspectives, reflection, and growth.
Who you are and your experience:
- Must have completed an accredited NP/PA program and be registered by the NYS Department of Education.
- 3 years or more experience practicing as a Nurse Practitioner or Physician assistant
- Experience working with a diverse and under-served patient population a plus
- Proven ability to work in a team environment
- eClinicalWorks experience is a plus

Pay: $140,000.00 - $170,000.00 per year (plus performance based bonus)

Preventive medicine services available at Tru Medical Management include cancer screening, smoking cessation, and weight loss. Specialized services include heart health and cardiology, podiatry, gastroenterology, physical therapy, and occupational therapy.
Patients at Tru Medical Management benefit from transparent pricing and don’t have to pay membership fees. The practice also welcomes most insurance. Consultations are available in person or via telemedicine, with a walk-in clinic for urgent care.
