Practice at : Fortuna Community Health Center – Open Door Community Health Centers

Nurse Practitioner/Physician Assistant assesses and evaluates the health care needs of patients, performs diagnostic and therapeutic procedures, institutes and manages care for patients, and performs related work as assigned.

Education & Experience

Diploma from an accredited program with credentialing through the American Nurses Association or Physician Assistant Association

Required Licensure & Certifications

Prior to employment, candidates must possess:

  • Current, unrestricted California license
  • Current DEA registration
  • Current Board Certification in specialty of practice (or board eligible if a newer graduate)
  • Current CPR/BLS certification
  • Valid California driver's license with an insurable driving record
