Position Overview

The Primary Care Physician will provide comprehensive outpatient care for patients across the lifespan while collaborating closely with an interdisciplinary care team to ensure high-quality, culturally responsive care.

Physicians will have a primary “home clinic” location in either Lake Forest or San Clemente, with flexibility to support nearby clinic sites as needed.

Clinical Responsibilities

• Provide full-spectrum outpatient primary care services
• Diagnose and manage acute and chronic medical conditions
• Coordinate specialty referrals and continuity of care
• Deliver evidence-based, patient-centered treatment plans
• Participate in quality improvement and collaborative care initiatives
• Support and mentor clinical team members as appropriate
• Educate patients on prevention, wellness, and chronic disease management

Practice Environment

• Average patient volume: approximately 18 patients per day
• Scheduled template supports manageable workflow
• Dedicated support staff including bilingual Medical Assistants
• Integrated referral and case management infrastructure
• Predominantly Spanish-speaking patient population
• Evening hours may be required occasionally, though schedules are designed to support work-life balance

Qualifications

Required

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in:
• Family Medicine (preferred)
• Internal Medicine
• Active California medical license or ability to obtain
• DEA certification or eligibility

Preferred

• Community Health Center or FQHC experience
• Spanish-speaking skills strongly valued
• Passion for underserved and multicultural patient populations
• Adaptability and strong team collaboration skills
• Comfort working in a mission-driven environment
