Responsibilities:
Provide direct, individualized nursing care to patients in private duty or home health settings
Assess, monitor, and document patient health status and changes in condition
Administer medications, treatments, and therapies as prescribed by the physician
Develop and implement personalized patient care plans in coordination with the healthcare team
Educate patients and family members on care procedures, medications, and health management
Communicate effectively with physicians, specialists, and other healthcare providers
Maintain accurate and timely clinical documentation and patient records
Requirements:
Active RN or LPN license in the state of Illinois
Previous experience in private duty nursing, home health, or a related clinical setting preferred
Strong knowledge of patient assessment, medication administration, and care planning
CPR/BLS certification required
Excellent communication, organizational, and critical thinking skills
Ability to work independently and manage time effectively in a home environment
