All functions shall be performed in accordance with the established policies and practices of the agency.

Essential Duties:- Guide and instruct the client in prescribing therapeutic self-care and create activities that are directed toward improving independence and physical and cognitive functioning
- Participate in the development of the plan of care
- Prepare a diagnostic evaluation report of each client and make recommendations
- Prepare clinical and progress notes
- Advise, consult and teach appropriate treatment procedures and supportive activities to clients, family member and other agency staff
- Establish household management routines
- Design and/or order appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning
- Instruct clients and their families in activities of daily living
- Attend care conferences as deemed necessary
- Instruct the family in the client's total occupational therapy program
- Maintains a high degree of confidentiality at all times due to access to sensitive information
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements
- Abides by all regulations, policies, procedures and standards
- Performs other duties as assigned
Position Requirements & Competencies:- Graduate of an accredited occupational therapy curriculum
- Certified as an occupational therapist by AOTA, American Occupational Therapy Association
- Current Occupational Therapist license in the state in which practicing
- Minimum one year of work experience as an Occupational Therapist
- Must have a valid Driver's license
- Must possess current CPR and First Aid Certification
- Must demonstrate ability to exercise independent judgment with limited supervision
- Must be comfortable working in a home health care setting within a client's residence
- Excellent verbal and written communication skills
- Flexible schedule, including availability to work evenings, weekends and holidays as needed
- Must be well organized and, excellent to time-management and attention to details skill
