PRN Physical Therapist (PT) – Skilled Nursing Facility

Schedule: PRN / As Needed
Location: Fort Thomas, KY

Join our rehabilitation team as a PRN Physical Therapist in a skilled nursing facility. Provide individualized care that helps residents improve mobility, restore function, and achieve greater independence in a collaborative, patient-centered environment.

Essential Duties and Responsibilities

• Evaluate residents’ strength, mobility, balance, functional abilities, and rehabilitation needs.

• Develop and implement individualized plans of care with measurable goals.

• Provide skilled physical therapy, including therapeutic exercise, gait training, balance activities, and functional mobility training.

• Monitor residents’ progress and adjust treatment plans as appropriate.

• Educate residents, families, and caregivers on safe mobility, home exercise programs, fall prevention, and equipment use.

• Collaborate with nursing staff, occupational therapists, speech-language pathologists, and other care team members.

• Complete accurate, timely evaluations, treatment notes, progress reports, and discharge documentation.

• Supervise Physical Therapist Assistants as assigned and in accordance with applicable requirements.

• Participate in discharge planning and recommend appropriate equipment and follow-up services.

• Follow facility policies, professional standards, and applicable state and federal requirements.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current Physical Therapist license in the state of practice.

• Strong clinical judgment, communication, and organizational skills.

• Ability to work independently and collaboratively with an interdisciplinary team.

• Skilled nursing or geriatric rehabilitation experience preferred.
