PRN Physical Therapist Assistant (PTA) – Skilled Nursing Facility

Schedule: PRN / As Needed
Location: Fort Thomas, KY 

Join our rehabilitation team as a PRN Physical Therapist Assistant in a skilled nursing facility. Work under the direction and supervision of a Physical Therapist to help residents improve mobility, restore function, and achieve greater independence.

Essential Duties and Responsibilities

• Provide skilled physical therapy treatments according to the plan of care established by the Physical Therapist.

• Guide residents through therapeutic exercises, gait training, balance activities, and functional mobility training.

• Monitor residents’ responses to treatment and communicate progress, concerns, or changes in condition to the supervising Physical Therapist.

• Educate residents, families, and caregivers on prescribed exercises, safe mobility, fall prevention, and equipment use.

• Collaborate with rehabilitation and nursing staff to support residents’ goals and continuity of care.

• Complete accurate, timely treatment documentation in accordance with facility policies and applicable requirements.

• Assist with discharge preparation and caregiver training as directed by the Physical Therapist.

• Maintain a safe treatment environment and follow infection prevention procedures.

• Follow professional standards, facility policies, and applicable state and federal requirements.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.

• Current Physical Therapist Assistant license or certification as required by the state of practice.

• Strong communication, organizational, and patient care skills.

• Ability to work effectively under a Physical Therapist’s supervision and within an interdisciplinary team.

• Skilled nursing or geriatric rehabilitation experience preferred.
