Job Description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.


 

Program Assistant – Adult Day

REQUIRED TO APPLY: Must be fluent in Cape Verdean Creole.

Wage: $16.00/hour
Schedule: Full-Time | Monday–Friday | Day Shift

No evenings or on-call shifts!

Join our Adult Day Health Program as a Program Assistant, providing compassionate care, engaging activities, and daily support to older adults and individuals with disabilities in a welcoming, community-based environment.

Qualifications

  • Must be fluent in Cape Verdean Creole
  • Must be at least 18 years old
  • High school diploma or equivalent
  • 1+ year of experience supporting seniors or adults with medical or rehabilitative needs
  • Knowledge of vital signs, weights, and activities of daily living (ADLs)
  • Comfortable assisting with personal care, brief changes, and transfers
  • Ability to pass background checks, physical exam, and TB screening
  • Compassionate, dependable, and strong communication skills

What You’ll Do

  • Provide daily care and support to older adults and individuals with disabilities
  • Assist with ADLs, personal hygiene, brief changes, and transfers
  • Monitor vital signs, weights, and participant well-being
  • Lead and support musical, social, and recreational activities
  • Encourage independence, social engagement, and participation
  • Maintain a safe, welcoming, and supportive environment
