Department Information

This position is open to current Colorado residents only.

Civil and Forensic Mental Health Services (CFMH) operates Colorado's two mental health hospitals, the Forensic Services Division, the Division of Mental Health Transitional Living, and now the Outpatient Services Division. The office provides a continuum of mental health care for both Civil and Forensic clients. Civil clients are individuals who are committed by a civil court into the care of the Colorado Department of Human Services because they have serious and persistent mental health conditions. Forensic clients include those who are charged with a crime and have a mental or developmental disability that prevents them from assisting in their own defense or rationally understanding the criminal court proceedings, and those who have been charged with a crime but found not guilty by reason of insanity. CFMH works across the legal system to provide evaluations, treatment and case management for these clients. Outpatient Services is the newest part of this continuum, working to support clients stepping down or being diverted from hospitals and jails, and giving them the care they need in the community.

Description of Job

This position is remote. The work location will be your home address, with on-location work and travel for in-person meetings, primarily in the Denver metro area.

What Youll be doing:

  • Responsible for employee supervisory responsibilities within the Outpatient Services Care Coordination Program.
  • This includes provision of clinical and administrative supervision, professional development, employee performance evaluations, employee performance plans, corrective actions, assists resolving informal grievances, staff scheduling, position descriptions, recruitment, retention, interviewing and hiring, onboarding, and training, and ensuring program-wide required training has been completed.
  • Facilitates new hire orientation, in collaboration with the Division of Outpatient Services (OSDV) and other programs. Lastly, this position is responsible for completion of Division-specific audits to support high quality documentation, assessment, and treatment.
  • Documents supervision, client contacts, and other required activities.
  • Responsible for the creation, implementation, and oversight of designated specialty programs and/or departmental projects. This may include implementation of processes in care coordination, case management, and court response and filing processes.
  • May also include partnering with community stakeholders for shared resources and development of discipline-specific training opportunities. At times, these duties require liaising with community stakeholders, community site visits, creating and sharing information materials or sessions, establishing protocol and policy, training staff, and overseeing and auditing the implementation and outcome measures.
  • Responsible for the oversight of all staff's progress with coordination of resource engagement with community partner agencies, CFMH clinical staff, and to oversee patient flow between jail or hospitals to the patient's longer term placement. This may include overseeing coordination of care and client referrals.
  • Must work closely with staff to monitor patient flow, in alignment with leadership.
  • Supports oversight of the team in identifying appropriate placement and planning in collaboration with the treatment team and liaisons with those placements to support continuity of care. In addition, this position and their team coordinate warm hand off meetings and attend off-campus appointments as needed.
  • Attends interagency staff meetings and is responsible for providing education to stakeholders regarding the Care Coordination Program, building relationships across various agencies.
  • As a supervisor, often supports team members to navigate various systemic barriers and ensure timely and accurate processes are completed. All efforts are geared toward supporting placement into the least restrictive environment.
  • Ensures staff maintain and enhance their professional knowledge and skills. This includes identifying training needs, providing coaching and mentoring, and ensuring compliance with continuing education and licensing requirements. This position will assess staff strengths and areas for development and growth through observation, supervision and performance review.
  • Develop professional development plans in collaboration with staff, aligning goals with hospital priorities and best practice standards.
  • Promotes staff involvement in committees, workgroups, and hospital quality improvement initiatives to foster growth.
  • Maintains up-to-date professional knowledge, licensure and clinical skills through ongoing continuing education and training.
  • Plan and facilitate in-service training on social work practices, hospital procedures, policies and compliance standards.
  • Prioritizes and decides which staff attend conferences, workshops, or hospital training within budgetary and schedule constraints
  • Other duties as assigned include ad hoc activities, in-clinic crisis response, group therapy coverage, milieu support, etc.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

SOCIAL WORK/COUNSELOR IV (C4L4XX)

Education/Licensure/Certification/Experience:

Current, valid Colorado licensure as a:

  • Licensed Social Worker (LSW) OR
  • Licensed Clinical Social Worker (LCSW) OR
  • Licensed Professional Counselor (LPC) OR
  • Licensed Marriage and Family Therapist (LMFT) OR
  • Licensed Addiction Counselor (LAC)

And two (2) years of licensed behavioral/mental health treatment experience in inpatient, outpatient, community-based, or jail-based settings.

Applicants for supervisory positions must have licensure that meets the legal requirements to perform supervisory responsibilities.

Preferred Qualifications:

  • 3+ years of clinical supervisory experience.

  • Experience with Serious Mental Illness.

  • Clinical Master's degree or higher.

Conditions of Employment:

  • 2-step TB (tuberculosis) screening is required of all new employees upon hires

  • Annual influenza vaccination

  • Other vaccinations as required by the State of Colorado

  • Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapy (LMFT).

  • Unrestricted ability to be credentialed with CMS Medicare and Medicaid.

  • Full background check and reference check required

  • This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.

  • Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.

  • Must possess a valid, non-restricted Colorado Driver's License or an non-restricted U.S. Drivers License for positions within 30 miles of the CO state border

  • Out-of-State Driver's License holders must obtain a valid, non-restricted Colorado Driver's License within 30 days of employment start date.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must:

  • Disclose that information on the application.

CDHS Selection Process Explanation:

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
