POSITION SUMMARY:
The Program Coordinator serves as a central operational support role responsible for coordinating grant-funded activities across Mobile Medical Unit operations, Remote Patient Monitoring (RPM), Community Health Workers, clinical teams, and community partners.
This position supports the organization’s mission as a Federally Qualified Health Center (FQHC) by improving access to care, care coordination, and patient engagement for underserved populations in rural communities, in alignment with HRSA Health Center Program requirements and approved grant objectives.
Hours: General daytime hours Monday through Friday and some weekends. This position is non-exempt and eligible for overtime in accordance with the Fair Labor Standards Act (FLSA).
The role focuses on implementation, coordination, tracking, and administrative support of program activities to ensure timely execution, accurate documentation, and compliance with grant, organizational, and regulatory requirements.
DUTIES AND RESPONSIBILITIES:
Program Coordination & Implementation
- Coordinate day-to-day implementation of Activity A and Activity B work plans, timelines, and operational tasks under direction of program leadership
- Support execution of Mobile Medical Unit outreach, rural care coordination, and RPM program activities
- Assist leadership in maintaining alignment with approved grant scope, deliverables, performance metrics, and reporting deadlines
- Coordinate scheduling, outreach calendars, Mobile Medical Unit deployment logistics, partner site coordination, and internal staffing communication
Outreach & Community Engagement
- Assist in organizing community outreach events, screenings, educational activities, and follow-up services in targeted rural areas
- Maintain collaborative relationships with community organizations, referral partners, and local stakeholders to support patient engagement and service utilization
- Support culturally competent outreach efforts aligned with the needs of underserved populations
Remote Patient Monitoring (RPM) Coordination
- Coordinate RPM workflows, including patient identification support, enrollment coordination, device distribution, tracking, and follow-up communication
- Facilitate communication between RPM participants and care team members to ensure continuity of care
Data Tracking, Reporting & Compliance
- Maintain accurate and timely records of program activities, outreach efforts, and operational workflows
- Support data collection, tracking, and reporting related to grant performance metrics, including patient engagement, referral completion, and chronic disease outcomes
- Assist in preparation of reports aligned with HRSA and Uniform Data System (UDS) requirements, as applicable
- Ensure all activities comply with organizational policies, HIPAA, and applicable federal, state, and grant requirements
Operational Support & Process Improvement
- Identify workflow barriers and communicate opportunities for process improvement to program leadership
- Assist in maintaining operational readiness, including coordination of supplies, equipment, and communication tools
- Participate in team meetings, planning sessions, and grant monitoring activities
- Perform other administrative and coordination duties as assigned

Required Experience:
POSITION REQUIREMENTS:
- Associate or Bachelor’s degree in public health, healthcare administration, social services, or related field preferred
- Minimum of 2 years of experience in community outreach, program coordination, or health education
- Experience in an FQHC, community health, or population health setting preferred
- Strong verbal and written communication skills with a customer service focus
- Bilingual (Spanish) preferred
- Proficient in Microsoft Office and general office technology
- Ability to coordinate multiple tasks in a structured and detail-oriented manner
- Valid driver’s license and ability to travel locally
- Access to a registered and insured vehicle for work-related travel
