Description
We are seeking a Program Manager Care Management to join our team at Independent Living Systems (ILS). ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations.
About the Role:
The Program Manager Care Management is responsible for overseeing and coordinating multiple health-related programs to ensure they meet organizational goals and regulatory requirements. This role involves strategic planning, resource allocation, and continuous evaluation to optimize program effectiveness and impact on community health outcomes. The Program Manager will lead cross-functional teams, manage budgets, and develop proposals to secure funding and support program sustainability. Effective communication with stakeholders, including healthcare providers, community partners, and internal teams, is essential to align program objectives and deliverables. Ultimately, this position drives the successful implementation and growth of health programs that improve service delivery and support vulnerable populations.
Minimum Qualifications:
Bachelor's degree in Business Administration, Public Health, Health Care Management, or a related field.
At least 5 years of experience in program management within the health care or social assistance industry.
Strong written and oral communication skills demonstrated through proposal development and stakeholder engagement.
Relevant experience may substitute for educational requirement on a year-for-year basis.
Preferred Qualifications:
Master's degree in Public Health, Health Administration, or a related discipline.
Familiarity with federal and state health care regulations and funding mechanisms.
Demonstrated success in securing grants and managing multi-year funded programs.
Responsibilities:
Develop, implement, and monitor program plans to achieve defined health care and social assistance objectives.
Manage program budgets, ensuring efficient allocation of resources and adherence to financial guidelines.
Lead proposal development efforts to secure funding from government agencies, foundations, and other sources.
Coordinate with internal teams and external partners to support program activities and resolve operational challenges.
Create tools to track program progress, timelines, and deliverables.
Prepare detailed reports and presentations to communicate program status, outcomes, and recommendations to stakeholders.
Respond proactively to crises or unexpected issues affecting program delivery, implementing mitigation strategies as needed.
Ensure compliance with relevant health care regulations, policies, and best practices.
