Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:
We have an exciting opportunity to join our team as a Project Facilitator - Brooklyn.

In this role, the successful candidate The Family Health Centers at NYU Langone is seeking a full-time Digital Navigator. The Digital Navigator works under the supervision of the Digital Education Coordinator and Project Coordinator for the Center for Community-Oriented Virtual Primary Care and Technology (CARE Tech).

Job Responsibilities:

Provide patient training on computer/mobile usage for access to medical and community information, including assisting patients with accessing and utilizing MyChart.

Provide patient training on Remote Patient Monitoring (RPM) device set up and use.

Develop patient-facing materials (i.e. infographics, brochures, etc.) on digital health related skills and distribute to patients as needed.

Distribute information to patients on affordable internet connectivity programs; assist patients and their families with enrollment via digital platforms.

Advocate for patients by identifying and addressing barriers to care, including language, health literacy, digital literacy, technology access, transportation, insurance coverage, childcare, and other social determinants that affect access to care.

Assist patients with accessing a full range of medical, behavioral health, community, and dental services.  

Attend staff and vendor-led technology trainings as scheduled.

Attend weekly scheduled status meetings with the site coordinator(s), CARE Tech Project Coordinator, and Digital Education Coordinator

Assist FHC staff in developing evidence-based interventions to increase MyChart activation at pilot site clinics.

Assist FHC staff in evaluation of project objectives.

Performs other duties as needed.
 

 

Minimum Qualifications:
To qualify you must have a High school diploma, GED, or equivalent. Candidate should have one year of experience in a health- or community service-related field and experience in community settings. Excellent computer knowledge and skills (Microsoft Office). Experience in simple data analysis. Effective oral, written, and interpersonal communication skills. Empathetic, reliable, resourceful, and a strong desire to help people. Proficiency in English.

Preferred Qualifications:
2 years of work and/or volunteer in community-based clinical or social service delivery. Familiarity with community-based organizations, community leaders, and community resources. Flexibility to work evenings and weekends. Available to travel throughout South Brooklyn. Ability to juggle priorities and manage time; work well under pressure. Strong organizational skills. Strong communication skills to work with people of diverse backgrounds.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $42,783.14 - $57,876.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
