I. JOB SUMMARY The Provider Services Nurse Practitioner (NP) at Big Sky Home Health & Hospice plays a vital role in supporting the organization's mission of providing life-changing service by helping patients receive primary care services as they navigate a complex medical system. This role focuses on clinical service delivery, provider support, care coordination, and operational excellence across Big Sky's care continuum.
The Provider Services NP collaborates closely with the Executive Director, clinical leadership, and interdisciplinary teams to ensure high-quality, cost-effective, timely, and compliant care delivery. All services are provided within the NP's scope of practice and aligned with organizational standards, regulatory requirements, and best practices.
II. DUTIES & RESPONSIBILITIES Under the supervision of the Executive Director, provide clinical support and services that promote safe, effective, and efficient healthcare delivery across Big Sky's service lines as a primary care physician. Support operational clinical workflows, provider services initiatives, and care coordination efforts to ensure continuity and quality of care. Deliver clinical services as appropriate via in-person visits, telephone, or telehealth encounters across multiple care settings, including residential, facility-based, and home environments. Serve as a clinical resource to interdisciplinary teams, supporting evidence-based practice, quality improvement initiatives, and best-practice standards. Perform comprehensive patient evaluations including history taking, assessment, diagnosis, treatment planning, prescribing medications, patient education, and evaluation of care outcomes. Collaborate with physicians, advanced practice providers, nursing staff, care coordinators, and operational leaders to optimize patient outcomes and service delivery. Ensure compliance with state-specific standards of practice for Nurse Practitioners and all applicable federal, state, and local regulations. Document clinical findings, assessments, and care plans accurately, thoroughly, and timely in the electronic medical record. Maintain strict patient confidentiality and safeguard protected health information at all times. Participate in provider services initiatives, clinical protocol development, and quality assurance activities as assigned. Maintain a safe working environment for self, patients, families, and staff by following infection control standards and utilizing appropriate personal protective equipment (PPE). Engage collaboratively with internal teams and external partners including healthcare providers, community resources, patients, and family members. Represent Big Sky Home Health & Hospice professionally at all times, fostering a cooperative and respectful work environment through tact, diplomacy, and a positive attitude. Demonstrate professionalism by being reliable, autonomous, accountable, and adaptable to changing clinical and operational needs. Manage workload effectively, prioritize responsibilities appropriately, and utilize work time productively. Maintain humility, openness to feedback, and willingness to accept coaching, constructive criticism, or corrective action when necessary.
