JOB SUMMARY: Responsible for providing nursing services to consumers

ESSENTIAL FUNCTIONS:

  1. Conduct medical and/or mental health assessments and evaluations.
  2. Administer and monitor medications. 
  3. Coordinate medical care and follow-up.
  4. Conduct home visits, sick visits and first-aid.
  5. Maintain consumer medical records.
  6. Provide medication education/nutritional counseling for consumers, families and staff.
  7. Consult with other program staff regarding consumer health issues and medication.
  8. Serve as liaison and advocate to community service providers for on-going, emergency and other forms of medical, mental health, crisis care, etc. for consumers
