Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered.

" Minimum of 2 years experience as a PT in an acute/home care/rehab setting.
" Required certs: BLS
" The service area is Yolo County. Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, Esparto. These are all the territories that we typically service.
" Good oral and written communication skills; has good organizational skills.
" Scrubs are not required, if scrubs are not worn the dress code is slacks (khakis are fine) and polo or button up shirt. Any color of scrubs is fine if scrubs are chosen.
" Usually rotate PTs on Saturdays only and once a month is the frequency.
" Provides physical therapy services to patients/clients according to a written physician's plan of care. This may include, but is not limited to:
o Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
o Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM - potential for rehab. Directing physical therapy treatment.
o Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy.
o Reporting the patient's/client's reaction to treatment or changes in condition to the physician.
o Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient/client, as well as instructing them as to the goals of the physical therapy program for the patient/client by participating in case conferences.
o May train patient/client in the use of prosthetic device.
o Supervising homemaker/Home Health Aides when such personnel are participating in patient's/client's rehabilitation regime.
o Identifies patient/client and family/caregiver needs for other home health services and refers as necessary.
o Prepares and submits clinical and progress summaries based on the attainment of goals, observing, recording and reporting information on the patient's condition to the attending physician and in the patient's health record.
o Provides physical therapy consultation to home families/caregivers when indicated.
o Participates in peer consultation process.
o Participates in and completes ongoing staff competency assessment.
o Complies with and understands Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and licensure requirements as they pertain to their position.
o Performs other duties as assigned and/or requested.

**Mileage is billable per GSA guidelines. Personal timesheets showing mileage must be signed by home health supervisors. There are meetings twice a month to have hard copies signed (if they do not see supervisors on a regular basis). They may also send photocopies of timesheets with mileage listed to the supervisors via email for approval. Timesheets need to be sent to *** in order to bill.
**Accrued mileage begins after the clinician departs from their first assignment. Travel between the clinician s housing and the first HH visit does not qualify for reimbursement.
