Stern Therapy Consultants is seeking a PRN Physical Therapist at Bezalel Rehab and Nursing Center in Far Rockaway, NY. Provide hands-on rehabilitation for skilled nursing residents, tailoring care to their medical needs, functional abilities, and goals for everyday movement.

What You'll Do

• Evaluate residents' walking, transfers, balance, strength, and range of motion to identify rehabilitation needs.

• Establish individualized treatment plans with measurable goals for recovery and daily function.

• Deliver therapeutic exercise, gait training, and mobility interventions appropriate to each resident's condition.

• Recommend assistive devices and teach residents and caregivers safe techniques for moving throughout their environment.

• Monitor outcomes, update plans of care, and provide clinical supervision to PTAs.

• Document evaluations and treatment sessions, sharing recommendations with nursing and the rehabilitation team to support care continuity and discharge planning.

Qualifications

• Active New York Physical Therapist license.

• Strong assessment skills and sound clinical judgment.

• Clear communication and accurate, timely documentation.

• Skilled nursing or geriatric rehabilitation experience is a plus; new graduates are welcome to apply.
