Description:
• We are seeking a Quality Navigator Registered Nurse to enhance patient outcomes through preventive care and chronic disease management.
• This role is based in Jamaica, NY and requires onsite presence.
• The position is contract-based and involves improving clinical quality by utilizing generic technology systems.
Requirements:
• BSN and active RN license in New York State
• Minimum 2 years of clinical experience
• At least 1 year of experience in an ambulatory/outpatient setting
• Experience with Medicare Advantage, managed care, or value-based care
• Knowledge of quality improvement programs
• Familiarity with chronic disease management and care coordination
• Proficiency in EMR systems and population health platforms
Qualifications:
• BSN and active RN license in New York State
• Minimum 2 years of clinical experience
• At least 1 year of experience in an ambulatory/outpatient setting
• Experience with Medicare Advantage, managed care, or value-based care
• Knowledge of quality improvement programs
• Familiarity with chronic disease management and care coordination
• Proficiency in EMR systems and population health platforms
