• Seeking a Quality Navigator Registered Nurse to enhance patient outcomes and clinical quality in a contract role located in New York, NY.
• The position involves supporting preventive care, chronic disease management, and advancing value-based care initiatives.
• Candidates must be proficient with electronic medical record systems and possess strong communication skills.
Requirements:
• BSN and active New York State RN license
• Minimum of 2 years of clinical experience
• Minimum of 1 year of ambulatory/outpatient experience
• Experience in Medicare Advantage, managed care, or population health
• Experience supporting quality improvement programs
• Knowledge of chronic disease management and care coordination
• Proficiency with electronic medical record systems
Qualifications:
• BSN and active New York State RN license
• Minimum of 2 years of clinical experience
• Minimum of 1 year of ambulatory/outpatient experience
• Experience in Medicare Advantage, managed care, or population health
• Experience supporting quality improvement programs
• Knowledge of chronic disease management and care coordination
• Proficiency with electronic medical record systems
